Short answer: The clear aligner treatment process begins with an orthodontic examination and a digital impression, after which the orthodontist plans the movement of the teeth in three dimensions. Once the aligners are produced, they are worn about 20-22 hours a day, and you usually switch to a new one every week or two. If necessary, attachments and IPR are applied, and regular checkups are carried out. When treatment ends, a retainer is used to keep the teeth in place; the total duration is determined by the orthodontist according to the case.
Many people considering straightening their teeth with clear aligners ask the same question before deciding: How will this journey unfold? The clear aligner treatment process consists of planned, consecutive steps, from the first examination to the post-treatment retention period. At our clinic, clear aligner treatment is carried out only with the Invisalign system; in this article we explain in order what is done at each stage, what is expected of the patient and which factors the duration depends on. If you are curious about the general framework of the method, you can also take a look at our clear aligner treatment page.
How are the first examination and digital impression done?
The process begins with an examination in which the orthodontist assesses the structure of the mouth and jaws in detail. During this appointment, the alignment of the teeth, the bite relationship, the condition of the gums and bone and whether there is a need for treatment of cavities or fillings are examined. When deemed necessary, panoramic and cephalometric X-rays are taken, along with facial and intraoral photos.
Next, the impression of the teeth is taken with an intraoral digital scanner instead of old-style impression materials. The tip of the scanner is moved over the teeth, and a three-dimensional model is created within a few minutes. For most patients this method is more comfortable, reduces the gagging sensation and increases the accuracy of the model.
Conditions that could prevent treatment are also addressed at the examination. If there is active gum inflammation, untreated decay or infected roots, these need to be resolved first. Clear aligners may not be suitable for every case; suitability is determined by evaluating the examination and records.
What do the treatment plan and simulation show?
After the digital records are collected, the orthodontist designs how the teeth will move step by step in Invisalign's planning software. This plan is prepared as a three-dimensional simulation showing, stage by stage, in which direction and how far each tooth will move.
The simulation allows the patient to see the goal of treatment in concrete terms; however, it should be thought of not as a promise but as the road map planned by the orthodontist. The biological response varies from person to person, and compliance with aligner wear directly affects the result. During the planning stage, the following become clear:
- The total number of aligners and the projected treatment time
- Whether attachments are needed and which teeth they will be placed on
- The need for interproximal reduction (IPR)
- Whether elastics are needed for bite correction
The plan is reviewed together with the patient, questions are answered and production begins once approval is given. The treatment fee is also communicated in writing at this stage, after the examination and planning.
What happens during aligner production and the first fitting appointment?
The aligners are custom-made according to the approved plan, and once they arrive at the clinic, the first fitting appointment is scheduled. At this appointment, attachments are bonded to the teeth if necessary, the first aligner is fitted and the orthodontist checks how the aligner seats.
The patient is shown in practice how to put in and take out the aligner. The aligners are usually pressed into place with the fingertips starting from the front teeth, and when removing them they are gently released from the back teeth. To learn daily wear habits in detail, our guide how to use clear aligners will be helpful.
A feeling of pressure in the teeth and a slight lisp in the first days are common. This feeling usually eases within a few days. It is recommended that the aligners be worn about 20-22 hours a day and taken out only while eating, having hot and sugary drinks and brushing the teeth.
How do aligner changes and checkup appointments work?
Each aligner is used for the period set by the orthodontist, after which you move on to the next one; in most plans this period is between one and two weeks. Feeling tightness for the first day or two after switching to a new aligner shows that the teeth have started to move in the planned direction.
Checkup appointments are usually held every few weeks. At these visits, the orthodontist looks at:
- Whether the teeth are progressing according to plan
- Whether the aligners fit the teeth fully
- Whether the attachments are in place
- Gum health and oral hygiene
At some point in treatment, some of the teeth may not have fully reached the planned position. In such cases, a new scan can be taken and fine-tuning with additional aligners can be planned. This is considered a normal part of treatment, and the decision is made based on the orthodontist's assessment.
Why are additional procedures like attachments and IPR needed?
Attachments and IPR are supporting procedures that help the aligners move the teeth more effectively and predictably. They may not be needed for every patient; the need is determined by the treatment plan.
What is an attachment?
Attachments are small bumps made of tooth-colored composite material. By creating a grip point for the aligner to hold the tooth, they make difficult movements such as rotation, extrusion or root movement easier. Because they are close to the color of the teeth, they do not draw attention, and they are removed at the end of treatment without damaging the tooth.
What is IPR (interproximal reduction)?
IPR is a procedure in which space is gained by removing a very small amount of enamel from the side surfaces of the teeth. The amount removed is kept within the safe limits of enamel thickness and is usually on the order of a tenth of a millimeter. In mild and moderate crowding, it can help create space without the need for tooth extraction.
In some cases, small orthodontic elastics are also used to correct the bite relationship. These elastics are attached to the aligners or to small hooks placed on the teeth and are effective with the patient's consistent use.
How long does clear aligner treatment take?
The duration of clear aligner treatment varies with the complexity of the case, and the exact time is determined by the orthodontist's examination. While limited cases correcting mild crowding or gaps can be completed within a few months, comprehensive treatments that also involve bite problems take between one and two years in most cases.
The main factors that affect the duration are:
- The degree of crowding and bite problems
- Compliance with daily aligner wear time
- Not missing checkup appointments
- Bone structure and individual biological response
- Whether additional aligners are needed
Wearing the aligners for fewer hours than recommended is the most common cause of delay. When the aligners are not worn, the teeth can tend to move back toward their old positions and the next aligner may not fit fully.
Why is a retainer important after treatment ends?
When treatment ends, a retainer, that is, a retention appliance, needs to be used so that the teeth can maintain their new positions. It takes time for the bone and connective tissue surrounding the teeth to adapt to the new position; if retention is not used during this period, the teeth can partially return to their old positions.
A retainer can take two forms: a thin fixed wire bonded to the inner surfaces of the teeth or a removable clear aligner-style retainer worn at night. In most cases, the two methods are used together. The removable retainer is to be worn for longer periods in the first months and then, as recommended by the orthodontist, only at night. For details on this topic, you can read our article retainers after clear aligners.
The retainer period is also followed up with checkup appointments. Retention is recommended to continue long term, and for most people indefinitely, because teeth continue to move slightly throughout life.

Frequently asked questions
Is other treatment needed before starting clear aligner treatment?
If there is untreated decay, gum inflammation or an infected root in the mouth, these need to be resolved first. Orthodontic treatment started on a healthy foundation progresses more safely. The orthodontist decides which procedures take priority after the examination.
How often do I need to change my aligners?
The aligner change interval is set according to the treatment plan and in most cases is between one and two weeks. Following the schedule recommended by your orthodontist is important for the teeth to progress according to plan. Stopping an aligner early or using it for too long should not be done without consulting the orthodontist.
How often do I need to come in for checkup appointments?
Checkups are usually scheduled every few weeks. At these visits, the progress of the teeth, the fit of the aligners, the attachments and oral hygiene are assessed. The interval is adjusted by the orthodontist according to the needs of the case.
How long should I use my retainer after treatment ends?
Because teeth continue to move slightly throughout life, long-term retainer use is recommended. It is worn more often at first and then usually only at night. The wear schedule is determined by the orthodontist at checkup appointments.
