What do people ask most about clear aligners?

The questions of patients considering clear aligner treatment fall under certain topics: how many hours a day the aligners are worn, how many months treatment takes, how they relate to eating and drinking, whether they are compatible with ablution (wudu) and prayer, whether they affect speech, at what age treatment can start, the factors that determine the cost, and problems such as losing or breaking an aligner. On this page we answer these questions in the categories 'General', 'Treatment', 'Wear', 'Cost', 'Suitability', 'Children and teens', 'Brands', 'Problems', 'Age', 'Orthodontic appliances', 'Invisalign' and 'Ankara / clinic'. The answers are short summaries of our guide content, which starts with our what are clear aligners page; for more detail, you can go to the relevant page.

How were these answers prepared?

We selected the questions from the patterns we encounter most often in search engines and at examinations. The answers are consistent with the patient information principles of the Turkish Orthodontic Society, the Turkish Dental Association (Türk Dişhekimleri Birliği) and the AAO (American Association of Orthodontists). In line with Turkey's health advertising regulations, there are no price figures, before-and-after photos or patient testimonials; cost is explained only through the factors that determine it. Each answer provides general information. Whether your teeth are suitable for clear aligners, how many aligners will be needed and how long treatment will take can only be determined after an examination and digital records.

Treatment

What is the shortest clear aligner treatment time?

In very mild cases, for example a small shift in a few front teeth or a slight relapse after braces, treatment can be completed in 3–6 months. Such short plans usually involve fewer than 10 aligners. However, even with short plans, a retainer must be worn regularly after treatment; otherwise, the teeth quickly move back to where they were.

Are clear aligners as effective as braces?

For mild to moderate crowding, spacing and mild bite problems, clear aligners give results similar to braces when properly planned. For major rotations, vertical movements and complex skeletal cases, braces are considered more predictable. Which method suits your case is determined by an orthodontic exam and digital planning.

How long does it take to close a gap between front teeth with clear aligners?

Generally, 4–6 months is expected for a single small gap and 6–12 months for a moderate gap or multiple spaces. The time depends on wearing the aligners 20–22 hours a day, the total amount of spacing and whether additional procedures, such as for the lip frenum, are needed. The exact plan is determined at the exam.

What happens at the first clear aligner appointment in Ankara?

At the first appointment, the orthodontist examines your teeth, gums and bite and requests a panoramic X-ray if needed. You are told whether clear aligners are suitable for your case and what the alternative options are. If you are a candidate, the iTero digital scan can be done the same day. The exact number of aligners and the duration are shared at a second appointment once the digital plan has been prepared.

Do I need teeth pulled before clear aligner treatment?

Most clear aligner cases do not require extractions; slight enamel reduction called IPR, or a slight expansion of the dental arch, is enough to create space. Extractions may be planned in cases of severe crowding or teeth that are tipped far forward. The decision to extract is made by the orthodontist after X-rays and digital planning.

Do you have to wear a retainer for life?

The current orthodontic approach recognizes that teeth shift slightly throughout life and recommends long-term retention. This means wearing a removable retainer every night, or at least a few nights a week, for life, or keeping a fixed retainer in place. Full-time wear is needed only in the first months; after that, only during sleep.

Should I get a dental implant first or clear aligners first?

The general rule is orthodontics first, then the implant. Once an implant is placed, it cannot be moved, and it limits the predictable movement of the surrounding teeth. Aligner treatment brings the gap to the correct width and corrects the opposing tooth, and the implant is placed afterward. In special situations such as insufficient bone, the doctors may change the sequence.

Will my result match the digital simulation exactly?

The simulation shows the goal of treatment; in most cases, the actual result is close to it but may not be exactly the same. The biological response of teeth varies from person to person, and wearing the aligners 20–22 hours a day directly affects the result. Small remaining differences are corrected with a refinement series. The simulation is a planning tool, not a promise of results.

Should the lip frenum be cut before closing a diastema?

If the lip frenum extends down between the two front teeth, a frenectomy may be needed. Today, most dentists prefer to do this procedure after the gap is closed or during treatment; an incision made too early can sometimes make gap closure harder because of scar tissue. Your dentist decides on the timing at the exam.

What is orthodontic camouflage treatment?

Camouflage is a treatment that masks a mild discrepancy between the jawbones by moving the teeth. For example, if the lower jaw sits slightly back, the upper front teeth are moved back so the teeth close properly; the jaw position does not change. It can be done with aligners or brackets. If profile expectations are high or the skeletal difference is large, camouflage is not enough.

Can protruding teeth be fixed without extractions?

Mild to moderate overjet can often be corrected without extractions using IPR, arch expansion and distalization. If the overjet is large, the lip profile is noticeably forward or crowding is also present, extracting the upper premolars may be considered. The decision is made based on X-rays, model analysis and a profile assessment.

How many aligners are there in total in clear aligner treatment?

Depending on the case, the number of aligners can range from 10 to 60, and more in some complex plans. Mild cases are planned with 10–20 aligners, moderate cases with 25–45, and complex cases with 45 or more. The number becomes clear once digital planning is complete; refinement series may be added later. The number of aligners is also one of the factors that affects the cost of treatment.

How long does Invisalign treatment take?

The duration depends on the complexity of the case and on wearing the aligners 20–22 hours a day. Mild cases can be completed in a few months; moderate and complex cases may take 12–24 months. Aligners are changed every 7–14 days according to the doctor's plan, and check-ups take place every 6–10 weeks. Your doctor gives you an estimated duration after digital planning.

What is the difference between clear aligners and braces?

Both methods move teeth by applying gentle, continuous force. Braces are bonded to the teeth and adjusted by the orthodontist; clear aligners are removable, made from a digital plan and changed by the patient. Aligners have advantages in terms of appearance and hygiene, while braces may be preferred for some complex movements and for patients who have trouble with compliance. The decision is made at the exam.

Can clear aligners widen a narrow upper jaw?

Aligners can widen the dental arch to a limited extent by tipping the side teeth outward; however, they cannot change narrowing at the bone level. Bone expansion is done with a rapid palatal expander during growth and with mini-screw-assisted or surgically assisted methods in adults. The type of narrowing is determined by model measurements and X-rays.

Do crowded teeth require extractions?

In mild to moderate crowding, extractions are usually not needed; IPR and arch expansion are enough. If the space shortage is large, the lip profile is not suited to being pushed forward or gum health is limited, extracting premolars may be considered. The decision is made after X-ray and impression analysis.

How many appointments does clear aligner treatment in Ankara require?

In a typical process, there are two to three appointments at the start for the exam, the plan consultation and aligner delivery. After that, check-ups are usually scheduled every 6–10 weeks; for a 12-month treatment, this means about six to eight check-ups. The number of appointments varies with the progress of the case and the doctor's plan.

What happens if you don't wear a clear retainer after braces come off?

Within weeks, the teeth start to shift back toward their original positions; rotated teeth turn back, closed gaps open up, and the lower front teeth begin to overlap. Within months, crowding can become noticeable. If caught early, returning to full-time retainer wear may be enough; with more advanced shifting, realignment with a short series of aligners is needed.

When do you start seeing results with clear aligners?

In the first weeks, the teeth do not move enough to notice in the mirror. For most people, the first visible improvement in the front teeth starts from around the second or third month; this is when gaps closing and crowding decreasing become noticeable. Comparing with your starting photos is the most reliable way to see the change. The complexity of the case affects this timeline.

Which is faster, clear aligners or braces?

Treatment time depends more on the complexity of the case than on the method. In mild cases, clear aligners can deliver results in a few months; in moderate and complex cases, both methods can take 12–24 months. With clear aligners, treatment time depends directly on wearing them 20–22 hours a day; if compliance drops, treatment takes longer. With braces, treatment time depends less on patient compliance.

What does a digital dental impression feel like?

An intraoral scanner is a small camera moved over the teeth; it does not cause pain, pressure or a gag reflex. The scan usually takes 5–10 minutes, and a 3D image of the teeth appears on the screen instantly. Clinics without a scanner may take silicone impressions; this is also a procedure that takes a few minutes and usually causes no discomfort.

Can patients from outside Ankara get clear aligner treatment?

Yes. Since clear aligner treatment requires fewer check-ups than braces, a suitable plan can be made for patients coming from out of town. The exam and scan can be completed in a single visit, and check-up intervals are arranged together with the doctor. However, attending every check-up appointment is necessary for treatment to progress as planned.

Can clear aligners be used before orthognathic (jaw) surgery?

In suitable cases, yes. Positioning the teeth correctly over the bone before surgery and fine-tuning after surgery can be done with aligners; additional devices are needed to stabilize the jaws during surgery. The orthodontist and the oral and maxillofacial surgeon decide together whether aligners or brackets are preferred.

Should gaps between teeth be closed with bonding or aligners?

If the teeth are of normal size, bringing them together with aligners gives a natural look and does not require altering tooth structure. If the teeth are small, bonding or veneers alone can make the teeth look disproportionately wide; in this case, positioning with aligners followed by a small bonding gives the most balanced result. Which path is suitable is determined by measuring tooth size.

Which is better: a fixed retainer or a removable retainer?

Both have advantages and limitations; most orthodontists recommend a fixed wire on the lower front teeth together with a removable clear retainer. A fixed wire does not depend on patient compliance but requires attention to hygiene and checks for breakage; a removable retainer covers all the teeth but doesn't work if it isn't worn. What is right for you is determined with your orthodontist based on your case and habits.

Only my lower front teeth are crooked. Can I get aligners for just the lower teeth?

In some selected cases, aligners can be used on one arch only. However, because the bite relationship with the upper teeth can change as the lower teeth are aligned, the dentist usually prefers to plan both arches together. Whether single-arch treatment is suitable is determined by a bite exam.

Does getting aligner attachments hurt?

No. Attachments are bonded to the tooth surface with composite filling material; there is no drilling or cutting of the tooth, and no anesthesia is needed. During the procedure, you only feel the teeth being dried and the material being hardened with a light. In the first days, the attachments may feel strange to your tongue; this feeling goes away quickly.

When will I see the first changes with clear aligners?

For most people, the first noticeable difference in the front teeth appears around months 2–4; gaps closing is noticed earlier than crowding being corrected. In the first weeks, the teeth move in steps too small to see in the mirror. Saving your starting photos and comparing them from time to time is the most objective way to see the change.

What is an iTero scan, and does it hurt?

iTero is an intraoral scanner made by Align Technology. Using a small camera wand, it captures a three-dimensional image of your teeth in a few minutes, with no impression material, gagging or waiting. The procedure does not cause discomfort, and at the end of the scan you can see a digital model of your teeth on the screen. Some clinics can also submit Invisalign cases using other scanners.

What determines how long Invisalign treatment takes?

The main factors are the complexity of the case, the quality of the planning and wearing the aligners 20–22 hours a day. Because Invisalign aligners are manufactured abroad, the first delivery and additional aligner sets can take a few weeks; this time is factored into the plan.

How often does a retainer need to be replaced?

A removable clear retainer usually wears out and is replaced every 1–3 years, depending on how intensively it is used and on teeth-clenching habits. Cracking, clouding, loosening or not fitting snugly are signs it needs replacing. A Hawley-type retainer can last for years. A fixed retainer stays in place as long as it doesn't break. At most clinics, replacement is charged separately.

Does IPR damage your teeth?

IPR involves thinning only a small portion of the enamel layer (usually 0.2–0.5 mm per tooth). Scientific studies show that when done correctly, it does not significantly increase the risk of cavities or sensitivity. Fluoride application after the procedure and regular flossing are recommended.

Does deep bite treatment make your front teeth shorter?

The front teeth are not shortened; the visible length of the tooth stays the same. The whole tooth is moved toward the jawbone (intrusion), so the gum level shifts along with the tooth. This movement is gradual and on the scale of millimeters; root length and bone level are monitored with X-rays.

Does the ClinCheck simulation show the actual result?

ClinCheck is a digital simulation showing the target position planned by the doctor. The actual result may differ somewhat from the simulation depending on your bone structure, how consistently you wear the aligners and how treatment progresses. That is why it is common to take a new scan toward the end of treatment and fine-tune the result with additional aligner sets (refinement).

Will closing a gap shift my dental midline?

In the planning software, the midline is designed to align with the facial axis; the gap is closed by distributing it evenly to both sides. If the midline is already off, correcting it during treatment can also be planned. For this reason, facial photos and measurements are taken before treatment.

Do clear aligners change your face shape?

Clear aligners move teeth; they do not change the shape of the jawbone, so a noticeable change in the face should not be expected. Moving protruding teeth back can slightly affect lip support and the profile seen from the side. Aligning the teeth directly improves the look of the smile. For jaw-related problems, clear aligners alone may not be enough.

Can an open bite close without treating tongue thrusting?

Aligners can close the teeth during treatment, but if tongue thrusting continues, the bite tends to reopen. For this reason, myofunctional exercises are started alongside treatment and a retainer is worn for a long time. A habit assessment is part of the treatment plan.

How long does it take for aligners to arrive?

The time from approval of the digital plan to the aligners arriving at the clinic varies by brand and place of manufacture. For systems produced in Türkiye, you can expect a few days to a week; for those produced abroad, two to four weeks. Your orthodontist will let you know the estimated delivery time for your plan.

Do clear aligners hurt more than braces?

With both methods, you feel pressure and tenderness as your teeth move; this is a sign that treatment is working. With clear aligners, the smooth surface reduces cheek irritation, and discomfort is usually felt in the first days of each new aligner. With braces, irritation from brackets and soreness after adjustments are more common. Individual experiences can vary.

What happens if treatment takes longer than expected?

Treatment taking longer is a common situation; it usually results from the need for a refinement series or from not wearing the aligners enough. The orthodontist takes a new scan and plans additional aligners. Whether additional aligners are included in the fee varies by clinic and brand; it is recommended that you find this out in writing at the start of treatment. Consistent wear reduces the chance of delays.

Why can teeth temporarily look worse during aligner treatment?

As teeth move, temporary gaps may open, the bite may not fit fully for a while and some teeth may move ahead of others. These intermediate stages are anticipated in the digital plan and are corrected as treatment continues. If an appearance or bite change bothers you, mention it to your dentist at your checkup; they will assess whether the plan is progressing as expected.

Can clear aligners widen a narrow upper jaw in adults?

Aligners can tip the side teeth outward to a limited extent, but they do not widen the bone. For narrowing at the bone level in adults, there are options such as mini-screw-assisted expansion (MARPE) or surgically assisted expansion (SARPE); after expansion, tooth alignment can be completed with aligners.

How often are check-ups during clear aligner treatment?

Check-ups are usually every 6–10 weeks; at each visit, how well the teeth are following the plan is assessed and the next set of aligners is handed over. Some clinics can extend the time between visits with remote monitoring apps. If an aligner doesn't fit, breaks or is lost, you should call the clinic without waiting for your scheduled check-up.

I have a deep bite and protruding teeth. Can both be fixed at the same time?

A deep bite and increased overjet (protruding teeth) often occur together and are addressed in a single plan. Usually the bite is opened first and then the upper front teeth are moved back; elastics and, if needed, distalization of the back teeth are added to the plan. If the lower jaw sits back, a skeletal assessment is done.

Can braces and clear aligners be used together?

Yes, a hybrid approach is used in some cases. For example, braces are used for a short period first for certain movements that are difficult with aligners, and treatment is then completed with clear aligners. In other cases, auxiliary appliances and elastics are added during aligner treatment. Your orthodontist makes this decision based on the needs of your case.

Are attachments necessary when treating crowding with aligners?

Attachments are often necessary to move rotated or out-of-line teeth. Because they are small tooth-colored composite bumps, they are not noticeable from a distance, and they are polished off at the end of treatment. In very mild cases, treatment can be planned without attachments.

Can a skeletal open bite in adults be fixed without surgery?

A mild skeletal component can sometimes be compensated without surgery by intruding the back teeth with mini-screw support; this is called camouflage treatment. For a significant skeletal open bite, orthognathic surgery is needed for a permanent result. Which path is suitable is determined by a lateral skull X-ray and clinical evaluation.

Will my teeth go back after treatment?

If a retainer is not worn, the teeth tend to shift back toward their original positions; this is called relapse, and it is especially noticeable in cases of gaps (diastema) and rotated teeth. The retainer is worn all day for the first months, then at night, and used regularly over the long term. With regular retention, the result achieved is maintained.

Is it better to close a missing tooth gap or get an implant?

Both are valid options, and the choice depends on the location and width of the gap, whether there is crowding, your age and the amount of bone. Closing the gap is often preferred for small gaps and cases with crowding, while an implant is often preferred for large gaps in the back of the mouth and when the bite relationship needs to be preserved. The decision is made by the orthodontist together with the implant dentist if needed.

Can a diastema reopen after it is closed?

Without retention, the gap commonly reopens. For this reason, at the end of treatment, bonding a fixed wire behind the front teeth and wearing a removable retainer at night are recommended. If causes such as tongue thrusting or the lip frenum have been addressed, the likelihood of relapse drops significantly.

How long does deep bite treatment take?

It can take about 9–12 months in mild cases, 12–20 months in moderate cases and longer in cases with a skeletal component. Because vertical movements progress slowly, wearing the aligners 20–22 hours a day is important. The exact time is estimated after digital planning and may be updated during treatment.

What is a refinement, and why is it needed?

A refinement is an additional series of aligners made from a new scan when the teeth have not fully reached the planned position by the end of the first aligner series. It is common in moderate and complex cases, because the biological response of teeth varies from person to person and the way the aligners are worn affects the result. It is not a treatment failure but a fine-tuning stage.

How long do you wear a retainer after clear aligner treatment?

For the first 3–6 months, the retainer is worn all day except during meals, and after that only at night. Because teeth tend to shift slightly throughout life, long-term wear a few nights a week or every night is recommended. Your orthodontist sets the schedule based on how stable your teeth are; when retainer wear is stopped, the risk of relapse increases.

Do teeth move while wearing a retainer?

A retainer is designed not to move teeth but to hold them in their current position; a properly fitting retainer keeps the teeth in place. With slightly shifted teeth, a retainer may bring them back toward their previous position, but this effect is limited. If there is noticeable shifting, a retainer is not enough; an orthodontist's assessment is needed for realignment.

Does everyone need refinement aligners?

No, but most moderate and complex cases do. When the first series is finished, the teeth are usually very close to the target; for the small remaining differences, new impressions are taken and a few additional aligners are made. This is a natural result of the biological response of teeth varying from person to person. It is a good idea to find out at the start of treatment whether refinements are included in the fee.

Can clear aligners close the gap from an extracted molar?

Molar gaps are large, and the tooth behind has to be moved forward together with its roots; this movement is challenging for aligners, takes longer and usually requires mini-screw or bracket support. In most cases, preparing the gap for an implant is the more predictable approach. Suitability is determined by X-ray and model analysis.

How long does clear aligner treatment take?

It can take 6–9 months for mild cases, 12–18 months for moderate cases and up to 24 months for complex cases. The main factors that determine the duration are how much the teeth need to move, the number of aligners, daily wear time and attending check-ups regularly. Your orthodontist will tell you the estimated duration after digital planning.

Can a crossbite come back after it is corrected?

Because the teeth lock into the correct bite, relapse is relatively low; however, an expanded arch may tend to narrow. For this reason, a retainer is worn at night for a long time, and causes such as mouth breathing are kept under control. Arch width is monitored with regular checkups.

Do teeth go back after clear aligner treatment ends?

If a retainer is not worn, the teeth tend to shift back toward their original positions; this is called relapse. For this reason, a retainer is given at the end of treatment: it is worn all day for the first months, then at night, and regular long-term use is recommended. Since teeth are known to shift slightly throughout life, retention is a permanent part of treatment.

How long do you wear a retainer after open bite treatment?

Because open bites have a high tendency to relapse, it is recommended to wear the retainer full-time for the first months and then at night for life. A clear retainer that also covers the back teeth helps limit the teeth from extruding again. Your dentist updates the wear schedule at checkups.

Can crowding come back after clear aligners?

Without retention, crowding commonly returns, especially in the lower front teeth. At the end of treatment, a fixed wire or a removable retainer is recommended, and nighttime wear continues for a long time. Retainer integrity is monitored with regular checkups.

Can I get fillings or a dental cleaning during treatment?

Yes. Since the aligners are removable, tartar cleaning and small fillings can be done during treatment. Procedures such as large fillings, crowns or extractions that change the outer shape of a tooth can affect how the aligners fit, so they should be planned with your orthodontist in advance. It is recommended that you continue your routine check-ups with your dentist.

What is distalization, and can aligners do it?

Distalization means moving the upper molars backward to make room for the front teeth. Aligner systems perform this movement sequentially, starting with the backmost tooth, and usually give predictable results of up to 2–3 mm. Elastic support, and sometimes mini-screws, are needed; the status of the wisdom teeth is evaluated beforehand.

How does clear aligner treatment work?

The process starts with an orthodontic exam. Next, a digital scan and X-rays of the teeth are taken, and the doctor plans the target position of the teeth digitally. Once the aligners are made, the first set is fitted and attachments are bonded if needed. Aligners are changed every 7–14 days according to the doctor's plan, and check-ups take place every 6–10 weeks. When treatment is complete, you switch to a retainer.

How many aligners are used in clear aligner treatment?

The number of aligners varies by case; 10–20 aligners may be planned for mild cases and 30–60 or more for moderate and advanced cases. Each aligner moves the teeth a small step forward. At the end of treatment, fine adjustments are made with an additional aligner set (refinement) if needed. The exact number of aligners is determined by digital planning and shared with you at the plan consultation.

What are attachments, and why are they used?

An attachment is a small tooth-colored composite bump bonded to the surface of a tooth so that the aligner can grip the tooth better and apply certain movements (such as rotation or moving a tooth up or down). Which teeth will get attachments is determined in the digital plan. When treatment is complete, the attachments are removed without damaging the enamel, and the tooth surface is polished.

What is IPR, and does it damage teeth?

IPR (interproximal reduction, also called stripping) is the removal of a very thin layer of enamel (usually 0.2–0.5 mm) with a polishing strip or bur to create space between teeth. Because it stays within the enamel thickness and is applied carefully, it is considered not to damage the teeth. Whether it is needed, and how much, is determined in the digital plan; it is not done in every case.

How do clear aligners move teeth?

Each aligner is made slightly different from the current position of the teeth (one step closer to the target). When the aligner is worn, it applies a light, continuous force to the teeth; this force triggers the bone surrounding the tooth to remodel on one side and form new bone on the other. As the aligners are changed in sequence, the teeth move along the planned path.

How often are clear aligner check-ups?

Check-ups are usually scheduled every 6–10 weeks. At each check-up, the doctor assesses whether the teeth are progressing according to the digital plan, how well the aligners fit and the condition of the attachments, and the next aligners are handed over. Needing fewer check-ups than with braces is convenient for patients coming from out of town. The frequency varies with the case and the doctor's plan.

What is a refinement (additional aligner set)?

A refinement is an additional set of aligners prepared from a new digital scan when the teeth have not fully reached the target position by the end of the first aligner series. It may be needed because of small deviations, gaps in wear time or unpredictable tooth movements. In many treatments, planning one or more additional sets is normal and is considered part of the treatment.

What happens after clear aligner treatment ends?

When the teeth reach the target position, active treatment ends, the attachments are removed and the retention phase begins. Because teeth tend to move back to their original positions, a retainer is used; it is recommended to wear it all day for the first months and at night after that. Retention is lifelong. A fixed wire bonded behind the front teeth is another option.

Children & Teens

At what age can kids start clear aligners?

Treatment can start with aligners designed for children during the mixed dentition stage, usually between ages 6 and 10. Orthodontic associations recommend a first evaluation around age 7; not every child needs treatment. Conditions such as a narrow jaw, crossbite and significant crowding may benefit from early intervention. Suitability is determined at the exam.

How can I tell if my child is wearing their aligners enough?

Aligners for children and teens have a compliance indicator: colored marks on the back of the aligner fade the longer it stays in the mouth. If the mark is still clearly visible when it is time to switch, the aligner has not been worn enough. In addition, a new aligner seating easily, a consistent daily wear routine and keeping the aligner in its case are signs of good compliance. At checkups, the dentist compares tooth movement against the aligner.

From what age can you wear clear aligners?

They can be used from adolescence, once most of the permanent teeth have come in; some systems have separate children's programs for the mixed dentition stage. For adults, there is no upper age limit; what matters is the health of the teeth and gums. For children and teens, suitability is assessed at an orthodontic exam.

Up to what age does SGK cover orthodontic treatment?

SGK (Turkish Social Security Institution) reimbursement for orthodontic treatment generally applies to patients under 18 and is subject to specific indication criteria. For adults, reimbursement is limited to exceptional cases. Age and indication criteria are set by the Health Implementation Communiqué and may be updated; it is advisable to get current information from the institution before applying.

Are clear aligners better than braces for teens?

Both are effective for teens. Clear aligners offer advantages in appearance, hygiene and activities such as sports; there are teen products with compliance indicators and eruption allowances. However, the responsibility for wearing them 20–22 hours a day lies with the teen. If there are concerns about compliance, fixed braces may be the safer option. Significant skeletal problems may require additional appliances; the decision is made at the exam.

Do aligners that move the lower jaw forward work for kids?

Aligners with mandibular advancement features work on a principle similar to functional appliances and can correct the bite when used during the growth spurt. Studies show that most of the effect is dental, while the skeletal contribution is limited but may be present. Success depends on wearing the aligners 20–22 hours a day and on timing.

My child has a crossbite. Does it need treatment right away?

A crossbite is one of the main conditions for which early orthodontic treatment is recommended. Especially when a jaw shift is present, it can be treated between ages 7 and 10 with expansion or simple appliances. An early orthodontic exam (around age 7) is enough to determine whether treatment is needed.

My child has gaps between the front teeth. Is treatment needed right away?

Between ages 7 and 9, a gap between the upper front teeth is a normal part of development and usually closes when the canines erupt. Only causes such as an impacted extra tooth, a very thick lip frenum or missing teeth are evaluated early. As orthodontic associations recommend, an orthodontic exam around age 7 provides guidance.

My child's front teeth stick out. At what age should treatment start?

An orthodontic exam is recommended around age 7. If the front teeth are very far forward, a short course of early treatment at ages 8–10 may be considered because of the risk of fracture. If the lower jaw sits back, the main growth-guidance treatment is planned during the growth spurt between ages 10 and 14; during this period, functional appliances or aligner systems that bring the lower jaw forward can be used.

At what age can Invisalign First be used?

Invisalign First is designed for children in the mixed dentition stage, which usually corresponds to ages 6–10. There is no strict age limit; the deciding factors are the stage of dental development, jaw growth and the child's ability to wear the aligners 20–22 hours a day. Suitability is assessed during an orthodontic exam.

How long does early clear aligner treatment take for kids?

Early treatment during the mixed dentition stage usually takes 6–12 months and is often the first phase of a two-phase plan. This phase aims at jaw expansion, space maintenance and correcting oral habits. Once the permanent teeth have come in, usually around age 11–13, a second phase of treatment may be needed. The duration varies with the case and with compliance.

My child has an open bite. At what age should treatment start?

If habits such as thumb sucking or pacifier use stop by age 3–4, most open bites correct themselves. An orthodontic exam is recommended around age 7; during growth, appliances that guide vertical growth can be used. Aligner treatment is usually planned after most of the permanent teeth have erupted.

Clear aligners or braces: which is better for kids?

Both can be used. Because clear aligners must be worn 20–22 hours a day, the child's level of compliance is the deciding factor; they are used successfully in children who can take responsibility. If there are concerns about compliance, fixed braces may be the safer option. Some brands offer products designed specifically for children in the mixed dentition stage; suitability is assessed at the exam.

Which clear aligner brands are suitable for kids?

There are special products designed for use during the mixed dentition stage (when baby teeth and permanent teeth are present together); Invisalign First is one of them, and some other brands also have children's lines. These products include designs that leave room for erupting teeth. Whether they are right for your child is assessed at the exam based on the dentition stage and level of compliance.

My child has a congenitally missing tooth. When should clear aligner treatment start?

Detecting a congenitally missing tooth early makes planning easier. Since implants cannot be placed while growth is ongoing, the options of closing the gap or maintaining it until growth is complete are considered. Aligner treatment is usually planned after most of the permanent teeth have come in; an orthodontic exam around age 7 provides guidance.

At what age can clear aligners be used?

There is no strict minimum age for clear aligners; some systems offer children's series used during the mixed dentition stage (ages 6–12, when baby teeth and permanent teeth are present together). More commonly, treatment starts once most of the permanent teeth have come in, around age 12–13. Suitability is assessed during the exam based on dental development and the child's ability to wear the aligners consistently.

Can kids use clear aligners?

Yes, in suitable cases. Aligner series designed for the mixed dentition stage aim to guide jaw development and address space problems early. During this stage, the aligners are planned to accommodate the loss of baby teeth and the eruption of permanent teeth. With family support, the child can wear the aligners 20–22 hours a day; the decision is made during the exam.

Are clear aligners as effective as braces for teens?

In well-selected cases, clear aligners can achieve results similar to braces in teens. Some systems offer compliance indicators on the aligners that show wear time for teens, and designs that leave room for erupting teeth. Braces may be preferred for complex bite problems. Which option is suitable is determined during the exam and in the digital plan.

Is there an age limit for clear aligners in adults?

No, there is no upper age limit. Adults of any age with healthy teeth and gums can be evaluated for clear aligner treatment. With age, tooth movement may be a little slower, and gum recession, missing teeth or dental prostheses may affect planning. For this reason, overall oral health is assessed in detail during the exam.

What is mandibular advancement?

Mandibular advancement is a treatment for growing teens whose lower jaw is set back, aiming to guide the lower jaw forward. In some clear aligner systems, wing-like features added to the aligners serve this purpose, so teeth can be straightened and the jaw guided at the same time. It is used only in patients who are still growing and in suitable cases.

At what age should children have their first orthodontic exam?

For children, a first orthodontic exam is generally recommended at around age 7. By this time, the first permanent molars and incisors have come in, and possible problems in jaw and tooth development can be noticed. An exam does not always mean treatment; for most children, only monitoring is planned.

When do children need early orthodontic treatment?

Early orthodontic treatment is considered when growth-related problems such as a narrow jaw, crossbite or a mismatch between the jaws are identified. Not all crowding requires early treatment; in some children, it is better to wait for the permanent teeth to come in. The orthodontist decides whether it is needed through an exam and X-ray assessment.

What problems is Invisalign First used for?

Invisalign First is a clear aligner system that may be considered for misaligned teeth and some jaw development problems in children in the mixed dentition stage. It may not be suitable for every child; suitability is determined at the exam based on the stage of development in the mouth. For general information, see our Invisalign First for children page.

How can teens stay consistent wearing their Invisalign aligners?

For teens, consistency comes from wearing the aligners for the recommended time and attending check-ups regularly. Because the aligners include indicators that track how long they have been worn, the orthodontist can monitor compliance. Reminders from the family and keeping the aligners in their carrying case reduce losses; there is more information on our Invisalign for teens page.

What happens if a baby tooth is lost early?

If a baby tooth is lost early, the neighboring teeth may drift into the gap, and there may not be enough room for the permanent teeth. This can lead to crowding later on. Whether a space maintainer is needed is determined by an orthodontic exam.

General

What kind of website is seffaf-plak.com?

seffaf-plak.com is an information website of Funda Sarıkaya Oral and Dental Health Polyclinic in Çankaya, Ankara, explaining clear aligner treatment with Invisalign. In line with the rules on health care advertising, it contains no superiority claims, no price figures and no before-and-after images.

Are clear aligners really invisible?

Because the aligners are thin and transparent, most people don't notice them from conversational distance. However, they are not completely invisible: the tooth-colored attachments bonded to the teeth can be seen up close, and the aligner may become slightly cloudy over time. Regular cleaning and changing the aligners on schedule keep them looking clear.

Are Invisalign and clear aligners the same thing?

Invisalign is the name of a system used in clear aligner treatment, while clear aligners is the general name of the method. Because Invisalign has been in use for a long time, people often use the name as a synonym for the method. At our clinic, clear aligner treatment is provided with the Invisalign system.

Why aren't clear aligner before-and-after photos shared?

In Türkiye, health advertising regulations do not allow patients' before-and-after photos to be published; this rule protects patients' privacy and prevents results from being presented in a misleading way. For this reason, illustrative drawings and descriptions are used on this site. You can see the projected result for your own teeth at the clinic with a simulation prepared from your digital scan.

Can I book a clear aligner appointment via WhatsApp?

Yes. You can send a WhatsApp message to 0312 911 60 36. Just include your name, the days and time ranges that suit you, and the reason for your visit. The clinic team will get back to you during working hours to confirm your appointment. To keep your health data secure, we recommend sharing detailed medical information during the exam rather than by message.

What should I bring to my first orthodontic consultation?

If you have them, bringing records of previous orthodontic treatment, your X-rays and the retainer you use makes the assessment easier. You will also be asked about any medications you take regularly and your general health. Bringing an identity document is required for registration. Brushing your teeth before the exam makes the doctor's assessment easier.

Do public hospitals in Turkey provide orthodontic treatment?

Yes. Many public oral and dental health hospitals have orthodontists on staff, and fixed braces treatment can be provided to patients covered by SGK (Turkish Social Security Institution). However, because the number of specialists is limited, not every hospital has an orthodontics department, and waiting times can be long. You can check the situation in your province through the central appointment system (MHRS).

What is the information on this website based on?

The information is based on sources such as the Turkish Orthodontic Society, the AAO (American Association of Orthodontists), Ministry of Health publications and peer-reviewed scientific studies, and it is written in line with Turkish health care advertising regulations. The sources used are listed on the Sources page.

Can a deep bite cause TMJ pain?

A deep bite can place extra load on the jaw joint and muscles; however, not every deep bite causes pain, and not every joint pain comes from the bite. In patients with joint complaints, a joint exam is done before treatment, and the effect of opening the bite on the complaints is evaluated individually.

Is there parking at the clinic if I come by car?

Yes. The clinic is inside Park Oran Ofis, and the building's parking garage can be used. The Panora Mall parking lot right next door is another option. The clinic is easy to reach by car via Oran and Turan Güneş Bulvarı; the drive from Çankaya and Kızılay takes about 15–20 minutes, depending on traffic.

Does an open bite affect speech?

Yes, the gap between the front teeth can cause a lisp, especially with hissing sounds such as s and z. Speech usually improves once the bite is corrected; however, if the tongue habit persists, speech therapy support may be recommended.

Can I get clear aligners at a university dental hospital?

Some dental faculties may offer clear aligners as part of research projects or for a limited number of cases, but this is not a general service. Fees and scope vary by institution. At faculty hospitals, treatment is carried out by resident dentists under the supervision of faculty members, and there may be waiting lists.

Can I get a weekend appointment?

The clinic is also open on Saturdays from 09:00–20:00 and closed on Sundays. Because demand for evening and Saturday appointments is high, we recommend calling or sending a WhatsApp message a few days in advance. Clear aligner check-ups are usually short, so they can be scheduled after work hours.

Can I decide on my own treatment based on the information on this website?

The content on this website is for general information only; it does not make a diagnosis or recommend treatment. Whether clear aligners are right for you, how many months treatment will take and which system will be used can only be determined after an orthodontic exam and digital records. We recommend using the content to prepare for your consultation with your doctor and to ask the right questions.

Does the clinic offer clear aligners other than Invisalign?

At our clinic, clear aligner treatment is provided with the Invisalign system. If clear aligners are not suitable for your case, other orthodontic options such as fixed braces are explained during the exam, and the decision is made together with the doctor.

Can a crossbite cause jaw shifting and facial asymmetry?

With a unilateral posterior crossbite, the lower jaw may shift to one side when closing. If this shift continues during growth, it can contribute to asymmetric jaw development; for this reason, early correction is recommended in children. In adults, how much of the existing asymmetry will improve is evaluated at the exam.

How do I get to the clinic in Park Oran by public transportation?

The clinic is in Park Oran Ofis, next to Panora Mall. From Kızılay and Çankaya, you can take a municipal bus that runs along Turan Güneş Bulvarı, get off at the Panora Mall stop and walk a few minutes. Those coming by car can use the Park Oran and Panora parking garages. We recommend checking the EGO app (Ankara's public transportation app) for current route information.

How often is the content on this website revised?

Pages are revised at least once a year. In addition, the relevant pages are revised when a new regulation, a new recommendation from the Turkish Orthodontic Society or the AAO, or an important scientific publication comes out. Errors can be reported via the contact page.

Can any dentist provide Invisalign?

Invisalign offers a training and certification program for dentists who will use the system; after completing this training, a dentist can use it. The dentist's orthodontic knowledge and planning experience are decisive for the success of treatment. At our clinic, Invisalign treatments are planned and monitored by an orthodontist.

How do I know if my jaw problem is skeletal?

It cannot be determined with certainty by looking from the outside. The orthodontist determines the share of dental and skeletal components through a clinical exam, profile photos and by measuring bone angles on a lateral skull X-ray (cephalometric X-ray). This analysis is the key step that shows whether aligners alone will be enough.

How long is the wait for orthodontic treatment at public hospitals in Turkey?

Waiting times vary greatly by province and institution. Because the number of orthodontists is limited and demand is high, there may be waiting lists of several months for the exam and the start of treatment. University hospitals have similar waiting lists. Checking directly with the institution is the most reliable way to learn the current situation.

What happens if crooked teeth are not fixed?

Overlapping teeth create areas that are hard to clean with a toothbrush and floss, which can increase the risk of cavities and gum inflammation. In addition, an unbalanced bite can lead to wear. The health impact of crowding varies from person to person and is evaluated at the exam.

Do clear aligners change the color or shape of your teeth?

No. Clear aligners change only the position of the teeth; tooth color, tooth shape and the appearance of the gums stay the same. If procedures such as whitening, edge contouring or bonding are wanted, they are usually planned separately after orthodontic treatment is complete. You can discuss this with your orthodontist.

Does fixing protruding teeth change your profile?

When the front teeth are moved back, the upper lip also moves back somewhat and closing the lips becomes easier; the change in profile depends on how far the teeth are moved back and on lip thickness. A profile caused by a retruded lower jaw does not change with tooth movement alone; this requires surgery or growth guidance during the growth period.

What happens if a deep bite is left untreated?

For some people, it may not cause noticeable problems for years; however, the lower teeth touching the palate can lead to gum recession, wear of the front teeth and fatigue in the jaw muscles. The extent of the effect varies from person to person and is evaluated at the exam.

What are clear aligners?

Clear aligners are thin, transparent plastic orthodontic appliances custom-made to move the teeth gradually. A series of aligners is made from an impression taken by digital scanning; each aligner moves the teeth a little further than the previous position. Aligners are removable and are worn 20–22 hours a day, except while eating and brushing. Treatment is planned and monitored by an orthodontist.

Are clear aligners different from braces?

Both are orthodontic treatments that move teeth, but braces work as a fixed system with brackets and wires bonded to the teeth, while clear aligners are removable and hard to notice. Braces are more effective for some complex movements; clear aligners require the discipline of wearing them consistently. Which one is suitable is assessed during the exam based on the case.

Do clear aligners really straighten teeth?

Yes. Clear aligners are a treatment method whose effectiveness has been shown in the orthodontic literature; they are widely used for mild to moderate crowding, gaps (diastema) and some bite problems. The result depends on the complexity of the case, the accuracy of the plan and wearing the aligners 20–22 hours a day. Which problems can be solved with aligners is determined during the exam.

Are clear aligners noticeable?

Because clear aligners are thin and transparent, they are usually hard to notice at conversational distance. Attachments bonded to the teeth (small tooth-colored composite bumps) may be visible up close. If the aligners are not kept clean, they can turn cloudy and become more noticeable. Visibility depends on the aligner material, the number of attachments and cleaning habits.

Does clear aligner treatment hurt?

When you switch to a new aligner, you may feel pressure and mild sensitivity in your teeth for the first few days; this is a sign that the teeth are moving and usually eases within a few days. Compared with braces, aligners are less likely to cause sores inside the mouth. Severe or persistent pain is not normal; in that case, you should see your doctor.

Use & Care

Can you drink water with clear aligners in?

Yes, you can drink cold or room-temperature water with your aligners in. Hot water is not recommended because it can distort the aligner. All drinks other than water (tea, coffee, juice, carbonated and sugary drinks) should be consumed with the aligners out; afterward, rinse your mouth and put the aligners back in.

How long does it take to get used to clear aligners?

Most people get used to talking and putting the aligners in and out within 3–7 days; it usually takes 2–4 weeks for the aligners to become a natural part of daily life. A mild feeling of pressure for two or three days returns with each new aligner, but it is not as noticeable as with the first one. Patients with many attachments may take a little longer to get used to removing them.

Can you clean clear aligners with toothpaste?

It is not recommended. Most toothpastes contain abrasive particles; these create scratches on the aligner surface that cannot be seen with the naked eye. The scratches trap stains and bacteria, and the aligner quickly turns cloudy and starts to smell. Brushing the aligner with a soft brush and a small amount of unscented liquid soap is both more effective and safe.

Do you have to wear elastics for an open bite?

Most open bite plans use vertical elastics to bring the front teeth together. The elastics are worn for most of the day and replaced when you change aligners. If they are not worn for the time your dentist sets, treatment takes longer or the target bite cannot be achieved.

Is it OK to wear clear aligners 18 hours a day?

The generally accepted wear time is 20–22 hours. Consistently wearing them for only 18 hours can cause your teeth to fall behind the plan, the aligners to stop fitting and treatment to take longer. If you fall short on an occasional day, wearing that aligner for a day or two longer may be enough; if it happens regularly, tell your dentist so the plan can be adjusted accordingly.

What are bite ramps on aligners, and are they uncomfortable?

A bite ramp is a small plastic bump added to the upper aligner behind the front teeth. The lower front teeth touch this bump and the back teeth are temporarily separated, which speeds up opening the bite. In the first days, it may feel strange that the back teeth do not touch; most patients get used to it within a few days.

Can you wash clear aligners with hot water?

No. Aligners are made from a thermoplastic material that is shaped with heat; hot water, boiling or the dishwasher permanently distorts the aligner, and it will no longer fit your teeth. Use cold or lukewarm water for cleaning. Do not wear an aligner that has lost its shape; go back to your previous aligner and let your clinic know.

Are clear aligners hard to wear?

Putting in and removing aligners is learned within a few days; the real challenge is changing habits. The 20–22-hour daily rule, brushing after every meal and having to take the aligners out for any drink other than water change snacking and sipping habits throughout the day. Most users adapt to this routine within a month.

Can you sleep with clear aligners in?

Yes, aligners stay in while you sleep; sleep makes up a large part of the 20–22 hours of daily wear. You will not swallow an aligner or choke on it; it fits snugly on your teeth. In people who clench their teeth, aligners can wear down over time; if this applies to you, let your dentist know.

Can you use denture cleaning tablets on clear aligners?

Yes. Denture (false teeth) cleaning tablets and tablets made for clear aligners and retainers work in a similar way, and most dentists consider them acceptable. Follow the time on the package, use lukewarm water and do not leave the aligner in the tablet solution for longer than 15–20 minutes. Avoid products labeled as whitening or containing chlorine.

Will treatment go faster if I switch aligners more often?

No. Tooth movement depends on the bone remodeling, and this process cannot be sped up. Changing an aligner earlier than planned means moving to the next aligner before the tooth has finished moving, which leads to aligners not fitting and ultimately a longer treatment. Only your orthodontist can change the switching interval.

How long do you wear crossbite elastics?

Crossbite elastics are generally worn for most of the day until the teeth in crossbite reach the correct position, usually for a few months. They are only removed for eating and brushing and are replaced every day. Your dentist updates the wear period at checkups; irregular use prolongs treatment.

What are Class II elastics, and how long do you wear them?

Class II elastics are small rubber bands that run from the upper canine area to the lower molar area; they guide the upper teeth backward and the lower teeth forward. They are usually worn all day except when eating and brushing, and replaced every day. How long you wear them ranges from a few months to a year depending on the case; your dentist updates this at checkups.

Will the gap from a missing tooth show during treatment?

A tooth-colored filler (pontic) can be added inside the aligner to fill the gap visually; this is common practice for front teeth. After treatment, the same method is used in the retainer until the implant is placed. When the aligner is taken out, the gap is visible.

Do clear aligners affect your social life?

Because aligners are hard to notice at conversation distance, their effect on appearance is limited. The main impact is on eating and drinking routines: long meals and get-togethers over drinks require taking the aligners out and keeping track of wear time. Carrying an aligner case and a small toothbrush makes these situations manageable. After the first week, speech is normal.

Do I have to use chewies with aligners?

It is not mandatory, but most dentists recommend it. A chewie helps the aligner seat fully on your teeth; it is especially useful in the first days after switching to a new aligner and when the aligner is not fully seated in one area. Using it for 2–3 minutes a few times a day is enough. If you do not have a chewie, a clean cotton roll can temporarily do a similar job.

Should I switch to a new aligner at night or during the day?

Most dentists recommend switching to a new aligner at night before going to bed. The tight feeling of the first few hours goes unnoticed while you sleep, and because the aligner stays in without interruption all night, your teeth adapt to their new position more quickly. If your dentist has told you a different timing, follow that.

Can I eat with clear aligners in?

No, the aligners are taken out while eating. Eating with aligners in can deform and stain them and causes food debris to build up between the teeth and the aligner. After eating, it is recommended that you brush your teeth and put the aligners back in. The aligners also need to be taken out for drinks other than water.

Do I need to brush my aligners after every meal?

After each meal, rinsing the aligner with water and brushing your teeth is enough; brushing the aligner itself in the morning and evening is ideal. If you put the aligner in without brushing your teeth, food debris stays in contact with your teeth under the aligner and the risk of cavities increases. If you cannot brush while you are out, rinse your mouth thoroughly with water and brush at the first opportunity.

Can you play sports with clear aligners?

Yes, aligners can stay in during most sports without disrupting treatment. In contact sports, an aligner does not replace a mouthguard; your dentist can recommend a suitable guard to wear over it. In sports such as swimming and running, the aligner stays in; drinking water and rinsing your mouth afterward is enough.

How do you floss with a fixed retainer?

To get floss under the fixed wire, use a floss threader or floss with a stiffened end; the floss is guided under the wire and between the teeth. Alternatively, interdental brushes and a water flosser help clean around the wire. Because this area is prone to tartar buildup, a dental check-up and cleaning every 6 months is recommended.

How many hours a day should clear aligners be worn?

Clear aligners should be worn 20–22 hours a day. They are taken out only for eating, drinking anything other than water, and brushing. When daily wear time drops, the teeth fall behind the plan, the aligners stop fitting properly and treatment takes longer. Paying particular attention to wear time on aligner change days helps the new aligner seat on the teeth.

Can you eat with clear aligners in?

No. Clear aligners are taken out when eating. Eating with aligners in can deform or crack them and trap food debris inside the aligner, increasing the risk of cavities. After eating, brush your teeth and put the aligners back in; if brushing is not possible, rinse your mouth thoroughly with water. You do not need to take the aligners out to drink water.

Can you perform wudu (ablution) with clear aligners?

Yes. Since clear aligners are a removable appliance, they can be taken out during wudu (ritual ablution) and put back in after rinsing the mouth. Taking the aligners out for a short time does not affect the daily wear time of 20–22 hours. For questions about religious rulings, it is appropriate to consult the relevant religious authority, and for questions about using the aligners, your doctor.

Do clear aligners affect your speech?

For the first few days, you may have a slight lisp or notice a change in 's' and 'sh' sounds; this usually goes away within a week as the tongue gets used to the aligner. Reading aloud speeds up the adjustment. If the speech problem persists, the edge of the aligner may be irritating the gums; in that case, the doctor can adjust the edge of the aligner.

Can you smoke with clear aligners?

Smoking causes clear aligners to yellow and absorb odors; it can also negatively affect gum health and tooth movement. It is recommended not to smoke while wearing the aligners; if you do smoke, take the aligners out and rinse your mouth afterward. Removing them often reduces daily wear time. The treatment period can be a good opportunity to cut down on or quit smoking.

Can you drink tea or coffee while wearing clear aligners?

It is recommended to drink only water while wearing the aligners. Drinks such as tea, coffee, cola and fruit juice can stain the aligners, and sugary and acidic drinks can collect under the aligners and cause cavities and enamel damage. Hot drinks can deform the aligners. For these drinks, take the aligners out, then rinse your mouth and put the aligners back in.

How do you clean clear aligners?

Brush the aligners every morning and evening with a soft toothbrush and lukewarm water, and a small amount of clear liquid soap if needed. Hot water is not used because it deforms the aligners. Colored or abrasive toothpastes can scratch the aligners. Aligner cleaning tablets can be used a few times a week. When the aligners are out, they should be kept in their case, not left in the open.

How do you put in and take out clear aligners?

Place the aligner over the front teeth first, then press it onto the back teeth with your fingertips to seat it; biting on a chewie (silicone roll) makes sure it is fully seated. To take it out, start from the back teeth: gently lift the aligner from its inner edge with a fingertip and work your way forward. Biting the aligner into place or pulling it off from a single point can break it.

What is a chewie, and what is it for?

A chewie is a small silicone roll you bite on for a few minutes after putting in your clear aligners. Biting helps the aligner seat fully on the teeth and helps close the gaps between the teeth and the aligner, especially when switching to a new aligner. Using it a few times a day is recommended, especially on aligner change days. It is provided by the doctor or the manufacturer.

How often do you change clear aligners?

Aligners are usually changed every 7–14 days according to the doctor's plan. The change interval is determined by the case, the type of tooth movement and daily wear time. Changing an aligner earlier than planned means the teeth cannot keep up and the new aligner will not fit. Noting the change day on a calendar or using the manufacturer's app makes tracking easier.

Eligibility

Can clear aligners fix a recessed lower jaw?

Aligners cannot move the lower jawbone forward. In mild retrusion, the bite is compensated by moving the teeth (camouflage); in growing children, aligner systems with precision wings can help guide the lower jaw forward. In adults with significant retrusion, the permanent solution is orthognathic surgery. Which path is suitable is determined by cephalometric analysis.

Clear aligners or braces: which is better for an open bite?

For a dental open bite, clear aligners are a favorable option because they cover the back teeth and intrude them slightly; brackets, on the other hand, are strong at extruding the front teeth. Both methods can be used and are sometimes combined. For a skeletal open bite, neither method is enough on its own; appliances or surgery are needed. The choice is made at the exam.

Can clear aligners fix severely crooked teeth?

In severe crowding, aligners alone may not be enough; however, with space-creating methods (IPR, distalization and, if needed, extraction), many severe cases can also be planned with aligners. For difficult movements, short-term bracket support or combined treatment may be preferred. Which path is suitable is determined by digital planning and an exam.

One of my teeth has a crossbite. Are clear aligners enough?

A single-tooth anterior crossbite is one of the problems aligners correct most predictably. While the tooth is tipped forward, the bite opens slightly for a while. It is usually planned together with aligning the other teeth and takes a few months within the overall plan. The root position and available space are assessed at the exam.

Can clear aligners fix all types of crowding?

No. Clear aligners are effective for mild to moderate crowding. In cases of severe crowding, heavily rotated teeth and cases requiring extractions, they may not be enough on their own; in these situations, braces or hybrid plans (braces and aligners together) are used. The level of your case is determined at the exam with X-rays and a digital scan.

Is a deep bite better fixed with clear aligners or braces?

Both methods can be used for mild to moderate deep bite. Brackets are stronger at intruding the front teeth by a large amount, while aligners give predictable results with bite ramps and control of the back teeth. In severe cases, the two methods can be combined. Which is suitable is determined at the exam based on the amount of vertical overlap and facial structure.

Clear aligners or braces for protruding teeth?

Both methods are effective for increased dental overjet; aligners make distalization predictable by moving teeth one at a time, while brackets are strong in root control in extraction cases. During growth, aligner systems that bring the lower jaw forward can be an alternative to functional appliances. Which is suitable is determined at the exam based on the amount of overjet, the jaw relationship and age.

I have a dental implant. Can I wear clear aligners?

Yes. Because an implant is fused to the bone, it does not move; it is marked as a fixed point in the plan, and the other teeth are moved around it. The implant's position may limit some movements, so the plan is evaluated digitally in advance. If an attachment is needed on the implant crown, a special bonding method is used.

Can people with gum disease get clear aligners?

Orthodontic treatment is not started while gum disease is active; periodontal treatment is done first and the disease is brought under control. In adults with controlled periodontal disease, clear aligners can be suitable because of their light forces and ease of hygiene. Gum monitoring continues throughout treatment; the decision is made at the exam.

Is there an upper age limit for clear aligners?

No. Tooth movement is possible at any age as long as bone remodeling continues; clear aligner treatment is provided to people aged 50, 60, 70 and older. What matters is gum and bone health, existing restorations and the medications you take. At an older age, movement may be somewhat slower; the plan is adjusted accordingly.

Can I use clear aligners if I have a dental bridge?

Yes, they can be used; however, because a bridge is a single piece, its abutment teeth cannot be moved separately. The aligner either moves the bridge as a whole to a limited extent or leaves it in place. If the abutment teeth need to move independently, the bridge is temporarily sectioned and replaced at the end of treatment. The decision is made jointly by the orthodontist and the prosthodontist.

Can I get clear aligners instead of braces?

In many cases, yes. Clear aligners are an alternative to braces for mild to moderate problems, and their scope has expanded with auxiliaries such as attachments and elastics. However, some tooth movements are harder to control with aligners, and your orthodontist may recommend braces or a combination of the two methods. The decision is made after an exam and digital planning.

Can I get clear aligners if I have a dental implant?

Yes, in most cases it is possible. Because an implant is fixed in the bone, it does not move; it is defined as a fixed point in the plan, and the other teeth are moved around it. Bridges, on the other hand, connect several teeth and cannot move individually; your dentist may leave them out of the plan. These situations do not rule out clear aligners; they are taken into account in planning.

Does clear aligner treatment take longer for adults?

Bone remodeling may be a little slower in adults than in teens, but in most cases this difference amounts to no more than a few weeks. Adults generally wear their aligners more consistently, which offsets this difference. Gum health, bone density and missing teeth affect the duration more than age itself.

Can you wear clear aligners while pregnant?

Pregnancy does not prevent clear aligner treatment, and ongoing treatment is usually continued. However, gum sensitivity may increase during pregnancy, nausea can disrupt your aligner routine, and X-rays are postponed. If you are about to start treatment, your dentist will evaluate the timing with you and schedule more frequent gum checks.

Can clear aligners fix an overbite?

Much of what is commonly called a protruding upper jaw is actually forward-tilted teeth or a lower jaw that sits back. The dental component is corrected with aligners. Protrusion or retrusion at the bone level is addressed with growth-guiding appliances during growth and with surgery in adults. A cephalometric analysis shows which component is dominant.

Can people with TMJ problems wear clear aligners?

If you have pain, clicking or locking in the jaw joint, a joint assessment is done before treatment begins. Because clear aligners change the bite, they can affect symptoms; in some people they reduce the effects of clenching, while in others symptoms may increase. If needed, your dentist plans preliminary treatment and monitors the process closely. The decision is made at the exam.

Can you get clear aligners after 40?

Yes. Adults make up a large share of clear aligner users. The most common reasons are crowding that was never treated in youth, relapse, lower front tooth crowding that increases with age and positioning teeth before prosthetic work. Gum health and existing fillings and prostheses are taken into account in planning; skeletal problems may require collaboration with a surgeon.

My lower jaw sticks out and all my front teeth are in crossbite. Will aligners work?

With a mild lower jaw protrusion, the bite can be compensated by moving the teeth with aligners (camouflage). With significant skeletal protrusion, tooth movement does not correct facial appearance; for a permanent solution, orthodontics is planned together with growth-guiding appliances during growth, or with orthognathic surgery in adults. Which path is suitable is determined by cephalometric analysis.

Can clear aligners fix jaw structure?

Clear aligners move teeth; they do not change the size or position of the jawbones. For significant skeletal discrepancies, aligner treatment can be planned together with growth-guiding appliances in growing children or with orthognathic surgery in adults. Mild bite problems of dental origin, however, can be corrected with aligners. The source of the problem is determined with X-rays.

I have gaps between all my teeth. Are clear aligners right for me?

Generalized spacing can also be treated with aligners; however, because the back teeth need to be moved forward, the plan is more extensive and elastic support may be needed. If the spacing is caused by gum disease, periodontal treatment is completed first. Suitability and treatment time are evaluated at the exam.

Is there an age limit for clear aligners?

There is no upper age limit; any adult with healthy gums and bone can be a candidate. In children, aligners can be used with special products from the mixed dentition stage onward; for teens, aligners with compliance indicators are used. What matters is not age, but the dentition stage, oral health and the ability to wear the aligners 20–22 hours a day.

Do teeth move more slowly at an older age?

Generally, yes. Bone remodeling can slow down with age, and tooth movement may take longer than in younger patients. This does not rule out treatment; the doctor plans the aligner change interval and the overall duration accordingly. Periodontal health, existing restorations and any medications you take also affect the pace; these are assessed during the exam.

Who are clear aligners suitable for?

Clear aligners are widely used for conditions such as mild to moderate crowding, gaps (diastema), mild deep bite or open bite, and relapse after braces. Patients with healthy gums, treated cavities and the ability to wear the aligners 20–22 hours a day are suitable candidates. The decision on suitability is made after an orthodontic exam and digital records.

Who are clear aligners not suitable for?

In cases of severe skeletal jaw discrepancies, cases requiring extraction of impacted teeth, serious gum disease, and patients who cannot wear the aligners consistently, clear aligners alone may not be sufficient. Active cavities and untreated gum problems are addressed first. In these situations, braces or combined treatment may be recommended; the decision is made during the exam.

Can clear aligners correct jaw structure?

Clear aligners move teeth; in adults, they do not change the size or position of the jawbone. Bite problems caused by tooth position can be corrected with aligners, but marked skeletal discrepancies may require treatment combined with orthognathic surgery or treatment with braces. For teens who are still growing, some systems offer add-on features that can guide the lower jaw forward.

Can you use clear aligners during pregnancy?

Pregnancy is generally not an obstacle to using clear aligners; the aligners contain no medication or radiation. However, gum sensitivity may increase during pregnancy, nausea can make wearing aligners harder, and stages requiring X-rays may be postponed. Telling your doctor that you are pregnant or planning a pregnancy allows the timing of treatment to be adjusted accordingly.

Can clear aligners be used if I have missing teeth?

Yes, in many cases they can. The gap from a missing tooth can be maintained in the plan, space can be opened for an implant, or the gap can be closed in suitable cases. The aligner is made to cover the toothless area, and if needed, it is shaped with a tooth-colored filler for esthetic purposes. Which approach is suitable is determined during the exam and in the digital plan.

Can people with gum recession use clear aligners?

Gum recession alone is not an obstacle, but gum health must first be assessed and any active gum disease treated. In some cases, controlled tooth movement does not worsen the gum condition; even so, the direction and amount of movement are planned carefully. Joint follow-up with a periodontist may be needed.

Can people who had braces before get clear aligners?

Yes. When a retainer is not used after braces treatment, teeth can shift again (relapse); this is one of the most common cases treated with clear aligners. Because relapse is usually mild, treatment may be shorter. Wearing a retainer consistently after the new treatment is important.

Cost

Why aren't clear aligner prices published online?

In Türkiye, promotional activities of healthcare facilities are regulated by law, and advertising fees is restricted under these rules. In addition, the cost of clear aligners is a personalized plan fee that varies by case; a figure given without seeing the case would not be realistic. For this reason, fee information is given to you personally and in writing after the exam and planning.

Does SGK cover clear aligner treatment?

Clear aligner treatment is generally not covered by SGK (Turkish Social Security Institution) reimbursement. SGK's orthodontic reimbursement is limited to fixed braces treatment provided at contracted public institutions for specific indications, usually for patients under 18. Since regulations may change, consult SGK and the institution you plan to visit for definitive information.

What affects the cost of clear aligners?

The main factors are the complexity of the case, the number of aligners needed and the package type, the brand and place of manufacture, the dentist's specialty, whether digital scanning and planning are included, retention appliances, additional aligner sets and the frequency of checkups. The city where the clinic is located can also be reflected in the fee through operating costs.

Why are there no prices on the website?

In Turkey, regulations on the advertising of health care services do not allow price figures or promotional price statements. That is why the website only explains the factors that determine the cost. The fee is provided in writing by the clinic after the exam and digital planning. The minimum fee schedule of the Turkish Dental Association (TDB) is used as a reference.

Can you pay for clear aligners in monthly payments?

Because treatment spans several months, most clinics offer a payment plan. Common arrangements are an initial down payment with the remaining balance spread across checkups, or splitting the amount into monthly credit card payments. It is a good idea to ask in writing whether the payment plan changes the total fee and whether additional sets and retainers are included.

Are Turkish-made clear aligners more affordable than imported ones?

Aligners manufactured in Türkiye are generally priced more moderately because they do not include import, customs and shipping costs and are less affected by exchange rates; delivery times may also be shorter. However, cost is not the only criterion: the material, the planning software and the dentist's experience with that system should also be considered when deciding.

Does the TDB minimum fee schedule cover clear aligners?

The minimum fee schedule set each year by the Turkish Dental Association (TDB) also covers orthodontic treatments and serves as a floor intended to keep services from being priced below a certain level. The fee a clinic charges is set above this floor depending on the scope of the case, the system used and the dentist's experience.

Does private health insurance cover clear aligners?

Most private and supplementary health insurance policies exclude orthodontic treatment and clear aligners or list them as exceptions in limited dental packages. If your policy includes dental coverage, you should get written confirmation from your insurance company on whether orthodontics and clear aligners are covered.

Do refinement aligners cost extra?

It depends on the package type. In comprehensive packages, additional aligner sets are generally included in the fee; in mild and moderate packages, they may be limited in number or charged separately. Asking for the refinement terms in writing in your quote before starting treatment helps you avoid surprise costs.

When will I find out the cost of clear aligners in Ankara?

The fee is provided in writing once the exam and digital planning are complete. The number of aligners, the chosen system, the need for attachments and IPR, and the need for additional aligner sets are the main factors that determine the cost. An exact fee cannot be given by phone or message before the exam; the minimum fee schedule of the Turkish Dental Association (TDB) is used as a reference.

Is there a cost difference between clear aligners and braces?

With both methods, the cost depends on the complexity of the case, treatment length, the system used and the dentist's specialty. For clear aligners, the brand, package type and place of manufacture affect the cost; for braces, the bracket type (metal, ceramic, lingual) does. Lingual braces and comprehensive aligner packages generally cost more than standard metal braces. The fee is provided in writing after an exam.

Is the retainer included in the cost of clear aligner treatment?

It varies by clinic and package. Some packages include the first retainer set, while others charge for it separately. Because retainers are worn at night for life, it is also useful to ask about the terms for replacement sets. For more details, see our page on retention after clear aligner treatment.

What determines the cost of clear aligners?

The cost of clear aligners is determined by the complexity of the case, the planned number of aligners and treatment duration, the chosen aligner system, the need for attachments and IPR, the need for additional aligner sets, and retainers. The doctor's expertise and the clinic's digital infrastructure are also reflected in the fee. The fee is provided in writing once the exam and digital planning are complete.

Why won't clinics quote the cost of clear aligners over the phone?

Because the fee depends on the number of aligners and the requirements of the case, it cannot be determined without an exam and a digital plan. In addition, health care advertising regulations in Turkey do not allow price statements before an exam. That is why clinics provide the fee in writing after digital planning. The minimum fee schedule of the Turkish Dental Association (TDB) is used as a reference.

What is included in the cost of clear aligners?

The treatment fee usually includes the exam, digital scan, digital plan, the full aligner series, attachment placement, IPR, check-ups and removal of the attachments. Additional aligner sets, retainers and replacement of lost aligners may be included or charged separately depending on the clinic. It is recommended to ask in writing at the plan consultation what these items cover.

Can I pay for clear aligners in monthly payments?

Many clinics offer a payment plan or the option to split credit card payments for clear aligner treatment; terms vary by clinic. The payment plan is discussed at the plan consultation, where the treatment fee is determined. Private health insurance usually does not cover orthodontic treatment or covers it only to a limited extent; it is recommended to check your policy with your insurance company.

Do public hospitals in Turkey offer clear aligners?

Orthodontic treatment is provided at public hospitals and university dental faculties, but clear aligners are mostly not covered by SGK (Turkish Social Security Institution) and are not offered at most of these institutions. Some university faculties may provide clear aligners as a paid service or as part of research. For the current situation, you need to contact the relevant institution.

Brands

Which clear aligner brand is better?

There is no single right answer. Brands differ in materials, planning software, place of manufacture and the range of cases they cover; however, what really determines the outcome is the dentist's experience and the quality of the planning. For mild cases, most systems are sufficient; for complex cases, the system the dentist is experienced with is preferred. The right brand is chosen together at your exam.

What is the difference between Invisalign Comprehensive and Lite?

Lite (or Express) is a package designed for mild cases that can be resolved with a limited number of aligners. Comprehensive is an extensive package for complex cases with no limit on the number of aligners, and additional aligner sets are included for a certain period. Your doctor decides which package is suitable based on the amount of tooth movement your case requires.

Can you switch aligner brands mid-treatment?

Technically, yes; a new digital scan is taken and the remaining part of treatment is planned on a different system. However, because this requires new planning and a new manufacturing process, it can affect treatment time and cost. In situations such as changing dentists or moving, the most suitable path is decided together with your dentist.

Is it safe to order at-home clear aligners online?

Aligners sold remotely without an exam by a doctor and without X-rays can miss gum disease, cavities or root problems and can cause uncontrolled tooth movement. The Turkish Orthodontic Society and the AAO (American Association of Orthodontists) recommend that orthodontic treatment be carried out under a doctor's supervision. Clear aligner treatment is a medical process that includes an exam and regular check-ups.

Troubleshooting

What should I do if my clear aligner doesn't fit?

First, bite on a chewie over the area with the gap for a few minutes several times a day, and wear the aligner close to 22 hours a day. If it doesn't improve in two to three days, go back to the previous aligner and wear it for a few more days. If the poor fit lasts longer than a week, call the clinic; the orthodontist may extend the switching interval or take a new scan and plan a refinement series.

What happens if you lose a clear aligner?

Put in the previous aligner right away and call the clinic; going without an aligner lets the teeth shift back. Depending on the condition of the teeth, the orthodontist may let you move on to the next aligner early or order a replacement for the lost one. Making a new aligner can take from a few days to a few weeks depending on the brand, and some systems may charge an additional fee.

My clear aligner is cutting my tongue. What can I do?

You can very gently round off the irritating edge with a clean nail file; don't shorten it too much. Apply orthodontic wax or a protective oral gel to the sore area and rinse with warm salt water. If the same area bothers you with more than one aligner, show it to the clinic; the orthodontist will smooth the edge and may update the trim line for the following aligners.

Do clear aligners hurt?

When a new aligner is put in, a feeling of pressure and tenderness in the teeth lasting two to three days is normal; it shows that force is being transmitted to the teeth. Since there are no wires or brackets, sores on the cheeks and lips do not form. The discomfort is usually mild and lessens over time; for severe or persistent pain, you should see your orthodontist.

Can I keep wearing a cracked or broken clear aligner?

If there is a small crack and the aligner still fits fully, you can keep wearing it and let the clinic know. If the aligner has split in two or a piece has broken off, it cannot apply force correctly; put in the previous aligner and call the clinic. Do not repair the aligner with glue; adhesives are not safe for the mouth and distort the aligner's shape.

Why do my clear aligners smell, and how do I get rid of it?

The odor comes from a bacterial film that builds up on the aligner surface. Wrapping the aligner in a napkin, putting it back in after eating without brushing, and only rinsing it with water make the smell worse. To get rid of it, brush the aligner morning and night with liquid soap, soak it in a cleaning tablet two or three times a week, and brush your tongue as well. If the smell persists, talk to your dentist.

Do clear aligners hurt, and how long does it last?

With each new aligner, a feeling of pressure and tenderness lasting 2–3 days is normal; it shows that force is being transmitted to the teeth, and it lessens with later aligners. Switching to a new aligner at night makes the first hours easier. Throbbing pain in a single tooth, a loose tooth, swollen gums or pain lasting more than a week is not normal; in that case, contact the clinic.

I didn't wear my retainer for a few days and it feels tight. What should I do?

A tight feeling shows that the teeth have shifted slightly. If the retainer seats without forcing it, wear it all day except during meals for a few days; the teeth usually move back. If it doesn't seat or causes noticeable pain, don't force it; uncontrolled force can damage the teeth. In this case, contact the clinic; the orthodontist may plan a short series of aligners or a new retainer.

My clear aligners turned yellow. Does it affect treatment?

Slight yellowing does not affect treatment; the aligner keeps moving your teeth. Yellowing is usually caused by drinking colored beverages or smoking with the aligners in. Because aligners are changed every 7–14 days, the next aligner restores the clear look. If a retainer you have worn for months has yellowed, you can try a cleaning tablet and a vinegar solution; if that does not help, you can ask for a new one.

Is a speech problem from clear aligners permanent?

No. Because the aligner reduces the space between the tongue and teeth, sounds such as "s" and "sh" may change in the first days; most people adjust within 3–7 days. Reading aloud and continuing to talk without taking the aligner out speeds up the adjustment. If a noticeable problem lasts longer than two weeks, the length of the aligner edges can be checked at the clinic.

What happens if an attachment comes off when removing my aligner?

A lost attachment does not stop treatment, but it can disrupt the planned movement of that tooth. Keep wearing your aligner and let the clinic know; the attachment will be rebonded at your next appointment or earlier if needed. To prevent attachments from coming off, always remove the aligner starting from the back teeth, and do not pull it off from the front teeth.

Will treatment be disrupted if an attachment comes off?

A single attachment coming off does not stop treatment; keep wearing your aligner and let the clinic know. The attachment is rebonded as soon as possible or at the next check-up. If the time without the attachment gets longer, the planned movement of that tooth may be delayed. If more than one attachment has come off, it is appropriate to ask for an earlier appointment.

What should I do if I lose my clear aligner?

If you lose your aligner, call your doctor right away. In most cases, you will be told to go back to the previous aligner or move on to the next one to keep the teeth from shifting back; the doctor makes this decision. A lost aligner can be remade, but this may take time. Putting the aligner in its case every time you take it out prevents wrapping it in a napkin, the most common reason aligners get lost.

What happens if a clear aligner breaks?

If there is a small crack, you can usually keep using the aligner; if there is a large break or a piece has broken off, consult your doctor. The doctor may recommend switching to the previous or next aligner or may reorder the aligner. The most common causes of breakage are biting the aligner into place, pulling it off from a single point and eating with the aligner in.

What should I do if my clear aligners don't fit my teeth?

A gap between the aligner and the teeth shows that the teeth are behind the plan. First, increase your daily wear time to 22 hours and use a chewie. If it does not improve within a few days, let your doctor know without changing the aligner; you may need to go back to the previous aligner or extend the aligner change interval. If the problem persists, an additional aligner set may be planned with a new scan.

My clear aligners turned yellow. What should I do?

Yellowing is usually caused by drinking tea, coffee or colored drinks or smoking while wearing the aligners, and by insufficient cleaning. Clean the aligners with lukewarm water and a soft brush and use aligner cleaning tablets; avoid hot water and whitening toothpastes. Since aligners are changed every 7–14 days, the yellowing is not permanent; reviewing your habits for the next aligners is enough.

My clear aligner is cutting my tongue or cheek. What should I do?

A sharp edge on the aligner can irritate the tongue or cheek. As a temporary fix, orthodontic wax can be applied to the edge; for a permanent fix, the doctor smooths the edge of the aligner. Do not trim the edge yourself at home, as this can affect the fit of the aligner. If a sore does not heal or there is bleeding, it is recommended not to wait and to make an appointment.

What should I do if an attachment falls off?

An attachment falling off is not an emergency, but it needs to be rebonded for the movement of that tooth to progress as planned. Keep wearing the aligner and inform your doctor; the attachment will be replaced at the next check-up or at an earlier appointment. If more attachments keep falling off, your technique for putting in and taking out the aligners is reviewed.

Do clear aligners cause bad breath?

Bad breath can develop because saliva flow decreases while the aligners are in and bacteria can build up under the aligners. To prevent it, brush and floss your teeth after every meal, clean the aligners morning and evening, and drink plenty of water during the day. If the odor persists, there may be a cavity or gum problem; this is assessed during an exam.

Can clear aligners stain your teeth?

The aligner itself does not stain the teeth; staining comes from food and drink residue trapped under the aligner or from insufficient brushing. Plaque buildup around attachments increases the risk of white spots (early cavities). Brushing after meals, using fluoride toothpaste and drinking only water while wearing the aligners reduce this risk. Tooth surfaces are assessed at check-ups.

What happens if you stop clear aligner treatment halfway?

When treatment is stopped halfway, the teeth remain in their intermediate position and may gradually drift back toward their original positions; the bite relationship may be left incomplete. If you have to take a break, consult your doctor; temporary solutions such as wearing your last aligner at night to maintain the current position may be recommended. When you resume treatment, a new scan and aligner set may be needed.

Orthodontic Appliances

What is a palatal expander?

A palatal expander is an orthodontic device used when the upper jaw is too narrow, to widen the palatal bone sideways. It is usually anchored to the upper molars and is opened as planned using the screw in its center. Which device is suitable is determined by an orthodontic exam. For details, see our article on palatal expanders.

At what age is a palatal expander used?

In most cases, a palatal expander is used during childhood and early adolescence, while the palatal bone is still developing. During this period, the suture line in the middle of the palate is more flexible, so expansion is easier. The right timing is assessed at the orthodontic exam based on the child's bone development; our orthodontic appliances page provides general information.

Does a palatal expander hurt?

A palatal expander usually causes a feeling of pressure in the palate and teeth during the first days rather than pain. This feeling, which comes shortly after the screw is turned, goes away on its own in most patients. If the discomfort persists or a sore develops, you should let your orthodontist know.

How do you turn a palatal expander screw?

The screw is turned with the special key supplied with the appliance, in the direction shown by the orthodontist and at the prescribed frequency. Usually, the key is inserted into the hole at the front of the palate and pushed toward the back to make one turn. Doing the first turn together at the clinic makes it easier for parents to learn the correct direction and amount; the turning schedule varies from person to person.

Is it normal to have a gap between the front teeth with a palatal expander?

Yes, a gap forming between the upper front teeth during expansion is usually expected. This gap shows that the palatal suture is opening, and it is usually closed in later stages of orthodontic treatment. The size of the gap and its follow-up are assessed at orthodontic check-ups.

How long do you wear a palatal expander?

After the active expansion period, when the screw is turned, the palatal expander stays in the mouth for a while longer so the widened bone can settle. The total time varies by person and treatment plan; rather than a fixed period, it is determined through orthodontic check-ups. Since most palatal expanders are fixed, the child cannot remove them.

Is palatal expansion possible in adults?

Because the palatal bone has matured in adults, conventional expansion appliances are often not enough; for this reason, options are assessed individually. For goals such as creating space in the dental arch, other orthodontic methods may be considered. The appropriate method can only be determined after an exam and imaging; you can also take a look at our Invisalign for adults page.

Is a retention period needed after palatal expansion?

Yes, a retention period is usually needed after expansion so the width gained can settle. During this period, the same appliance is left in the mouth passively for some time, and then the next stage of treatment begins according to the orthodontist's plan. Because the length of retention varies from person to person, it is determined at check-ups.

What is a functional appliance (Twin Block)?

A functional appliance is a removable orthodontic device used to influence the position and growth direction of the lower jaw; the Twin Block is a common type. It consists of an upper and a lower part and guides the lower jaw into a more forward position when the child closes their mouth. Whether it is suitable depends on the stage of jaw development and the orthodontic assessment.

What is an orthodontic face mask, and who needs one?

A face mask is an orthodontic device worn outside the mouth, used in growing children whose upper jaw is set back, to guide the upper jaw forward. It is usually used together with an appliance inside the mouth. Who it is suitable for is determined through an exam and X-ray assessment; our article on face masks has more details.

What is a space maintainer, and what does it do?

A space maintainer is a small type of appliance used when a baby tooth is lost early, to keep the neighboring teeth from drifting into the gap. It helps keep the space open until the permanent tooth comes in. Which type of space maintainer is needed depends on where the tooth was lost and on the child's development; see our article on space maintainers.

What is a habit-breaking appliance, and does it work?

A habit-breaking appliance is a device worn inside the mouth to reduce the damage that habits such as thumb sucking or tongue thrusting can do to tooth and jaw development. It can help a child stop the habit, but its effect also depends on the child's motivation and the family's support. The decision to use one is made after an orthodontic exam.

How many hours a day should you wear a removable appliance?

In most cases, removable appliances are worn for the time recommended by the orthodontist, usually most of the day and during sleep. The exact number of hours depends on the type of appliance and the goal of treatment, so it is important to follow the wear time set for you. Regular wear directly affects how treatment progresses.

Is it hard to talk and eat with an orthodontic appliance?

In the first days, speech may sound a little different because the appliance feels like a foreign object in the mouth, and most people get used to it within a few days. For meals, removable appliances are usually taken out; with fixed appliances, it is recommended to avoid sticky and hard foods. Your orthodontist will give you personalized instructions for use.

Should you choose an orthodontic appliance or Invisalign First?

The choice between an appliance and Invisalign First is made at the orthodontic exam based on the child's age, jaw structure and the problem to be corrected; one cannot be said to always be more suitable than the other. For some goals, such as guiding jaw development, appliances come to the fore, while Invisalign First may be considered for some tooth alignment cases. For details, see our Invisalign First page.

What should you do if your orthodontic appliance is lost or broken?

If your appliance is lost or broken, the right step is to call the clinic rather than trying to fix it yourself. The orthodontist will decide whether the device needs to be replaced or repaired. Going without the appliance for a long time can affect treatment progress and how well the device fits; you can reach us through our contact page.

What is an orthodontic appliance?

An orthodontic appliance is a removable or fixed device worn inside the mouth that helps the jaws and teeth develop correctly. It is most often used in growing children. An orthodontist decides which device is suitable after an examination.

Is a gap between the front teeth normal during palatal expansion?

Yes, it is usually normal. As the midpalatal suture opens, a temporary gap can appear between the two upper front teeth, and this usually shows that the expansion is working. In most cases the gap closes over the following period; it is monitored at check-ups.

How many hours a day should a removable appliance be worn?

It depends on the type of device; for many removable appliances, the orthodontist recommends a set number of hours a day, usually long-term wear that includes nighttime. Not following the recommended wear time can make treatment take longer. The exact wear time is set by the treating orthodontist.

Does an orthodontic appliance affect speech?

In the first days there may be a slight change, especially with "s" and "sh" sounds, but most children adjust within one to two weeks. Reading aloud helps speed up the adjustment. If speech problems last longer, the orthodontist should be told.

What foods should you avoid with an orthodontic appliance?

With fixed appliances, avoid sticky foods such as chewing gum and caramel, and very hard foods such as ice and hard pits or seeds. Removable appliances are usually taken out while eating. The orthodontist gives diet advice specific to the device.

What should you do if an orthodontic appliance is lost or broken?

Contact the clinic instead of trying to repair the device yourself. A device that has not been worn for a long time may no longer fit properly, so do not delay the appointment. A broken or lost device may need to be replaced.

At what age should a child have their first orthodontic exam?

The American Association of Orthodontists (AAO) recommends a first orthodontic exam at around age 7. This exam does not necessarily mean treatment; for most children, monitoring is recommended. If needed, the right time for early intervention is determined. For details, see our page on what age is right for clear aligners.

Can adults use a palatal expander?

In adults, expansion is more complex because the midpalatal suture has matured. Options such as mini-screw-assisted expansion (MARPE) or surgically assisted expansion may be considered for suitable patients. These options can only be discussed after an orthodontic exam.

Which is more suitable: an orthodontic appliance or clear aligners?

They are not interchangeable methods; an appliance mostly focuses on jaw structure and growth, while clear aligners focus on the position of the teeth. In some plans the two are used one after the other. At our clinic, clear aligner treatment is provided only with Invisalign.

Invisalign

What is ClinCheck?

ClinCheck is the digital treatment plan in Invisalign treatment that shows the planned movement of the teeth in 3D. The orthodontist reviews the plan, modifies it if needed and approves it; you can also see the steps of the movement. The plan may not match the actual result exactly; it can be updated at check-ups. For general information, see our Invisalign page.

What is an iTero scan, and does it replace dental impressions?

iTero is a system in which the teeth are captured digitally with an intraoral scanner, and in most cases it removes the need for conventional impression material. The scan is used to prepare the ClinCheck plan. Most patients find it more comfortable than having impressions taken; even so, suitability is assessed based on the structure of the mouth.

How does orthodontic treatment with Invisalign start?

The process starts with an orthodontic exam; the orthodontist examines the mouth and teeth and takes the necessary records and scan. A digital treatment plan is then prepared and approved, and the aligners are manufactured. For an overview of the process, see our clear aligner treatment page.

What does Invisalign Provider mean?

An Invisalign Provider is a clinic or dentist that is authorized to use the Invisalign system and has been trained in it. Treatment is planned and monitored by the treating orthodontist. Our clinic is an Invisalign Provider; for details, see our what are clear aligners page.

Do I need a dental cleaning and fillings before Invisalign?

In most cases, the teeth and gums are expected to be healthy before treatment begins. If there is tartar, tooth decay or gum disease, you may be asked to have these treated first. What preparation is needed is determined at the exam.

To book an appointment, you can call 0312 911 60 36 or send a WhatsApp message to the same number. The clinic is open Monday–Saturday, 09:00–20:00. The first appointment is an orthodontic exam; if you are a candidate, the iTero digital scan can be done the same day. The fee is provided in writing once digital planning is complete.

What languages are spoken at the clinic in Ankara?

The clinic provides services in Turkish, English, German and Russian. Patients who live abroad or do not speak Turkish can hear about the exam and the digital treatment plan in their own language. For patients coming from out of town or from abroad, check-up intervals are arranged together with the doctor, taking advantage of the fact that clear aligners require infrequent check-ups.

Age & Special Cases

At what age is Invisalign First used?

Invisalign First was developed for children in the mixed dentition stage, when baby teeth and permanent teeth are present together; in most children this stage falls between about ages 7 and 11. What matters is the stage of tooth development and jaw growth more than calendar age. Suitability is assessed with an orthodontic exam and X-rays.

Clear aligners or fixed braces for children?

Both can be used in early treatment. Because clear aligners are removable, brushing is easier and there are few food restrictions; however, they need to be worn 20–22 hours a day. Where the required wear time cannot be ensured, or for some skeletal problems, fixed appliances may be more suitable. The choice is made together with the family after the exam.

Will a child who had early treatment need a second phase?

Possibly. Early treatment aims to address problems that negatively affect jaw development in time. Once all the permanent teeth have come in, a second phase of treatment may be planned for any remaining irregularities. In some children the second phase is short, and in others it is not needed at all; this depends on growth and the type of problem.

How does a child use aligners at school?

The aligners are taken out during meals and placed in their case; after eating, the mouth is rinsed with water and the aligners are put back in. It is recommended to always keep the case in the school bag so the aligners don't get wrapped in a napkin and lost. Drinks other than water should not be consumed while the aligners are in. In the first weeks, a parent keeping track of wear time makes compliance easier.

At what age can teens start clear aligners?

In most teens, the permanent teeth have largely come in by around age 12, and clear aligner treatment can be planned at this stage. If some teeth are still coming in, space can be left for them in the aligner. The exact timing is determined at the exam based on the stage of tooth development and jaw growth.

What does a compliance indicator do?

A compliance indicator is a colored mark on the aligner that fades as it is worn. It gives an idea of whether the aligner has been worn enough; the orthodontist and family review it together at check-ups. The indicator alone is not an exact measurement; whether the tooth movements are following the plan is also checked.

What happens if a teen loses an aligner?

If an aligner is lost or broken, the clinic should be informed. Depending on the situation, the orthodontist may recommend going back to the previous aligner or moving on to the next one, and a new aligner is ordered if needed. To prevent loss, the aligner should always be put in its case when it is taken out.

Can you wear aligners while playing sports?

For most sports, you can keep wearing your aligners. For contact sports or situations where a mouthguard is needed, the orthodontist will explain the appropriate approach for you. If the aligners were taken out for sports, they should be put back in shortly afterward.

I had braces years ago and my teeth shifted. Can clear aligners help?

In most cases, yes. When a retainer is not used after braces, shifting of the front teeth is common, and these shifts can usually be corrected with clear aligners. Gum and bone condition are assessed at the exam; retention is planned after treatment so the same problem does not come back.

Is it hard to talk with aligners?

For the first few days, you may notice a slight change in some sounds. Most people get used to it quickly, and speech returns to normal. Reading aloud and continuing to talk with the aligners in helps you adjust faster.

Can clear aligners be used on teeth with crowns or fillings?

Yes, they can. On crowned teeth, the method for bonding attachments may be different, and this is taken into account in planning. Large fillings and root canal–treated teeth are also assessed. Implants cannot be moved; the plan is made accordingly.

Does treatment take longer for adults than for teens?

Not always. The type and extent of the problem determine the duration more than age does. Because bone tissue is more mature in adults, some movements are planned more carefully. The exact duration is shared once the digital plan is prepared.

Can you get clear aligner treatment after age 60?

Yes, it is possible. There is no upper age limit for orthodontic treatment. What matters is gum health and bone support. If there is gum disease, it is treated first; movements are then planned to suit the condition of the bone.

I have a dental implant. Can I get clear aligners?

Yes, you can. Because implants fuse with the bone, they cannot be moved; however, the other teeth can be moved while taking the implant's position into account. The aligners also cover the crown on the implant; the plan is prepared with the implants taken into account in the digital scan.

How are clear aligners planned for teeth with a dental bridge?

Teeth joined by a bridge act as a single unit and cannot be moved individually. The plan takes this into account; sometimes replacing the bridge after treatment may also be part of the plan. This decision is made together by the orthodontist and the prosthodontist.

Can people with gum recession wear aligners?

Yes, if gum health is under control. For teeth with gum recession and bone loss, movements are planned to be gentler and more controlled. Gums are monitored regularly throughout treatment, and good oral hygiene is important.
