This Privacy and Security Policy applies to the seffaf-plak.com website (the “Site”) operated by Funda Sarıkaya Oral and Dental Health Polyclinic (the “Clinic”). Personal data processed in connection with the health services provided at the Clinic is additionally governed by the relevant legislation and the Clinic's in-house privacy notices.

When you fill out the appointment form, you share information such as your full name, phone number, email address, who the examination request is for, the subject, your preferred contact method and your message.

- Contacting you to schedule appointments
- Keeping the Site secure and resolving technical problems

Information you submit through the form is not used for marketing purposes and is not sold to third parties.

Your personal data may be shared with the service providers used for hosting the Site and providing the form service, to the extent required by the service and under contractual safeguards. Information submitted through the appointment form is retained for as long as needed to conclude your request and for the periods set out in the relevant legislation; when these periods end, it is deleted, destroyed or anonymized.
