You saw the ad: you take your impressions at home and the aligners arrive at your door. It sounds easy. But this is a treatment that will stay in your mouth for years. Let us first look at what gets left out.
What is missing in a mail-order aligner system?
In these systems, a dentist often does not examine you at all. You take the impressions yourself, take photos and send them in. The plan is prepared remotely. The problem is this: there is a lot inside the mouth that cannot be seen by eye.
- No X-ray. Root structure, bone level, an impacted tooth, a cavity or a cyst cannot be seen in a photograph.
- No gum examination. Moving teeth in a mouth with gum disease can make things worse.
- The bite and the jaw joint are not assessed. Even if the teeth line up, the bite can be thrown off.
- A faulty impression goes unnoticed. If the impression taken at home is incomplete or distorted, the aligners will not fit.
- Follow-up stays weak. When an aligner does not fit, or a tooth does not go where expected, you may not know who to go to.
These are not small details. Orthodontics means moving a tooth within the bone. Planning without seeing the bone is like setting out on a road without a map.
The right answer to "where to buy"
A question often searched online: where can I buy clear aligners? The answer is not a shop or a website. Clear aligners are a treatment, not a product. The right address is a dentist who carries out an orthodontic examination. Before any aligners are ordered, the dentist looks inside your mouth, asks for X-rays, works out the condition of your teeth and jaws and tells you which method suits you. The aligners come after that.
The dentist may not find you suitable for clear aligners. That is also a valuable result. Some bite problems cannot be solved with aligners; in that case another method is suggested. If you ordered remotely, you would never get this information. We summarized the cases where aligners are suitable on the page who clear aligners are for.
Remote pre-assessment and remote treatment are not the same
Let us clear up one misunderstanding. Getting an initial opinion from a photograph is not a problem. Before deciding on treatment, sending a photo of your teeth and getting a general idea from a dentist is useful, especially if you are in another city. The limit begins here: a pre-assessment does not make a decision; it only points a direction. Making aligners requires a dentist's examination and records.
| Topic | Dentist-supervised treatment | Mail-order remote system |
|---|---|---|
| Mouth examination | In person, by the dentist | Often not done |
| X-ray | Requested when needed | Usually none |
| Measuring the teeth | Digital scan or clinical impression | Impression taken at home |
| Treatment plan | Prepared by the dentist and shown to you | Prepared remotely |
| If a problem comes up | Resolved with an appointment | It can be hard to find someone to turn to |
| Gum and cavity monitoring | Followed at every stage | Left to your own observations |
Common problems
In treatments that go ahead without dentist supervision, the following can be encountered: aligners that do not fit fully, a tooth that does not go to the planned place, a changed bite, gum recession, an existing cavity that advances unnoticed under the aligner. Not all of these happen to everyone. But dentist check-ups are the only way to catch these risks early. For common problems that come up during aligner treatment and their solutions, you can look at the page on clear aligner problems.
There is also the matter of timing. As treatment drags on, you may feel far from your dentist; yet the teeth move a little more every month and need to be monitored every month.
Price comparisons can also be misleading. The examination, X-rays, plan and check-ups are all part of a treatment. A package that leaves them out looks cheap, but the skipped steps show up later. Turn the question around: which dentist services are included at this price?
There is also the matter of timing in another sense. A tooth moving in the wrong direction can create work that takes months to correct. What looks like a saving at the start may later require planning from scratch with a dentist.
In practice it goes like this: a year later the teeth look lined up, but the bite does not meet properly. You load one side when chewing and your jaw joint gets tired. Or a cavity has grown unnoticed under the aligner. To see these early, you need an eye that looks inside the mouth.
What does the safe path look like?
- Examination. An orthodontist looks at your mouth and jaws and takes the necessary records.
- Plan. In what order the teeth will move and where they will go is shown to you on screen. You have the right to ask questions.
- Delivery of the aligners. The aligners are prepared according to your plan, and their fit is checked at the first delivery.
- Regular check-ups. Your mouth is examined at set intervals and the plan is updated if needed. For details, there is the page on retainers after treatment.
- Finishing and retention. The result is maintained.
This path also works if you live in another city or abroad. You have your first examination, receive the plan and have the check-ups at planned intervals. To get general information from photographs before you start, there is the online pre-assessment page. This is not a treatment decision; it only helps you look in the right direction.
Ask yourself before you decide
- Did a dentist look in my mouth, or did I only send photos?
- Do I have an X-ray?
- Did I see the plan on screen, and could I ask questions?
- If the aligner does not fit or I have pain, is there somewhere I can go?
- Do I know what will be done to keep my teeth in place at the end of treatment?
If your answer to even one of these questions is no, it makes sense to stop and book an examination appointment. For another view on why aligner systems should be considered together with dentist supervision, you can also read the article on dentist supervision and remote systems.
This information is general; the treatment that suits you is decided after an examination.
